Patients with bipolar spectrum illnesses - depressed patients without a clear history of mania but with a number of other markers of bipolar disorder - often seek help for their depressive symptoms and the interpersonal chaos caused by irritable, angry, and unstable moods. They are frequently misdiagnosed as having unipolar depression or personality disorders and mistakenly treated with antidepressants or psychotherapy alone. This seminar recording will provide you with a four-part, clinical diagnostic method to distinguish patients with personality disorders, unipolar depression, and attention-deficit hyperactivity disorder from those with bipolar illness. You will learn how to identify and mange individuals with alcohol abuse and those at greatest risk of suicide. You will also learn about potent non-drug treatments for bipolar disorder and the research showing why, despite widespread use, antidepressants, Lamictal and atypical antipsychotics should not form the foundation of treatment for bipolar illness. Drawing on research from three proven psychosocial treatments for bipolar disorder and psychodynamic studies on manic-depressive illness, Dr. Quinn will discuss the many ways psychotherapists can help patients and their families reduce the risk of relapse and re-hospitalization.

OBJECTIVES
  • Determine the two most important signs clinicians need to look for in patients’ histories that identify episodes of hypomania
  • Describe four lines of clinical evidence that must be assessed to identify patients with bipolar illness and to distinguish them from those with unipolar depression and personality disorders
  • Identify and manage alcohol and drug abuse
  • Identify and manage patients at high risk of suicide completion
  • Implement strategies derived from empirically-tested psychotherapies for helping bipolar patients recover
  • Recognize and understand the evidence showing why antidepressants, antipsychotics, and lamotrigine should not be the primary medications used for bipolar depression

OUTLINE

Recognition, Differential Diagnosis, Co-morbidity
  • Bipolar Disorder: Fad or Epidemic?
    1. What accounts for the dramatic increase in the diagnosis of bipolar disorder?
    2. Unipolar or bipolar depression? Why it matters
    3. No evaluation of a depressed patient is complete until you check for these seven symptoms of mania/hypomania
    4. Beyond symptoms: Why an evaluation of family history, course of illness, and response to antidepressants is critical to the diagnosis and treatment of mood disorders
    5. In the news: A teen commits suicide on Zoloft, a politician’s family history
  • Differential Diagnosis: Bipolar Disorder vs. Medical Illness, Cocaine Abuse, Borderline Personality, and ADHD
    1. What every psychotherapist needs to know: Clues to medical mimics
    2. Cocaine abusers lie (or at least omit the truth): Detecting covert drug abuse
    3. Bipolar or borderline? The intersection of trauma, personality, and mood
    4. Attention-deficit hyperactivity disorder and childhood bipolar illness
  • Other Problems Bipolar Patients Often Have (Co- morbidities)
    1. Why you must consider the diagnosis of bipolar illness in patients with panic and other anxiety disorders, eating disorders, and substance abuse
    2. Is my patient alcoholic? Don’t ask these two questions to find out
    3. How to increase your patient’s motivation to address addiction

Treatment
  • Psychotherapy
    1. Psychodynamics of euphoric and irritable hypomania
    2. Helping patients overcome lethargy, lack of motivation, and rumination
    3. Three evidence-based pychotherapies for bipolar disorder: Integrating core concepts into daily practice
    4. A key task for any therapist: Helping patients come to terms with the bipolar diagnosis and the need for medication
  • Fun Video Clips: Creativity, Comedy and Mania
  • Your Worst Fear: Identifying and Managing Suicide Risk
    1. Three risk factors that predict suicide completion within one year
    2. How to protect your patient and yourself (risk manangement 101)
    3. The only medication repeatedly shown to dramatically reduce suicide risk (not an antidepressant!)
  • Potent Non-Drug Treatments for Bipolar Disorder
    1. Light
    2. Darkness
    3. Manipulation of sleep
    4. Blueblockers
    5. Fish fat and other natural compounds
  • Medications
    1. Four myths: Antidepressants help protect bipolar patients from new episodes of depression; Lamictal effectively treats acute bipolar depression; atypical antipsychotics are mood stabilizers; lithium, Depakote and Tegretol don’t relieve depression
    2. Lithium: Still important – and still the best for many patients
    3. Depakote: It’s role in bipolar disorder and comorbid panic and anxiety
    4. Tegretol: Why it’s a great, but often overlooked, drug for young women
    5. Not just for MDs: Bipolar depression and suboptimal thyroid functioning
